Exit Forms
Please complete these two forms and submit them to the AmeriCorps*VISTA Manager when a member ends his/her service. Whenever possible these forms should be completed even if a member terminates service early.

 Exit Form  
VISTA completes section one. CNCS completes section two
 Future Plans Form  
VISTA and supervisor complete section A and B. CNCS completes section 3.
