NEW ONLINE PROGRESS REPORT

Beginning in October 2008, progress reports will only be accepted through FormSite, our online reporting system.  The purpose of this conversion to an electronic format is to streamline data collection and save paper.  We have changed the reporting elements slightly to better correlate with the information requested of us by CNCS.  If you have any questions related to how your site's data fits into this new model, please feel free to contact Katie at kjohnson@theMLC.org or 651-251-9151.

 

Here are a few tips for using the new online system:

 

Create one account per VISTA project

By creating an account with Form Site you can save your work and return as often as necessary to complete the report. For each quarter, you will need to create a new login and password.  If you have forgotten your password go to the FormSite home page and request a reminder.

 

Please create one account for each project that both VISTA and supervisor can use. If you create two accounts (i.e. one for the VISTA and one for the supervisor) MLC cannot tell which of the two forms is the official report. It will double the number of entries we receive. If you have more than one VISTA please create an account for each project so you have separate reports for each member.

 

How do I save my work?

FormSite automatically saves your work when you press the “next” button at the bottom of each page. Please note that FormSite will automatically log out if there is no activity on the page for an extended period of time. You work will not be saved if the site automatically logs you out, so plan accordingly.

 

Uploading documents

You will upload several documents to complete the progress report form. If you make a mistake, simply upload the correct document and FormSite will override your previous upload. You can only upload one document per attachment section. Please contact the AmeriCorps*VISTA Manager if you have difficulty with this feature.

 

Check your work prior to submission

The only required information on the online form is the contact information. All other fields remain optional so VISTAs and supervisors can complete the sections in non-sequential order. This means that the system will not issue a warning if you forget to complete a field. Please double check your work prior to submission to ensure that the report is complete.
